The main purpose of this study was to establish diabetes risk assessment model as well as its implementation and application. Intervention to prevent type 2 diabetes should be directed toward individuals at increase risk for the disease. Therefore, we developed the diabetes risk score system. According to the study of residents’ health census data in Liao-Shen area include the diabetics and non-diabetic patients, logistic regression analysis was adopted to screen the main risk factors of diabetes and establish the diabetes risk assessment model. This model was placed on the mobile terminal with backstage where the data could be analyzed achieving lifestyle interventions and exercise-habits proposals aimed at the selected high-risk diabetes. As a result, this model can be used to assess diabetes risk of residents in Shenyang community which is expected to become an effective tool in China.
